Expanding Care and Collaboration: Highlights from the Kings/Tulare SRN Alliance Meeting
Community leaders and service providers from Kings and Tulare Counties recently gathered virtually for the Smart Referral Network (SRN) Alliance Meeting, continuing their shared mission to build stronger, more connected communities through closed-loop referrals.
Tulare County SRN Update Fedieson Landicho, SRN Coordinator for United Way of Tulare County, shared that the Tulare SRN system has officially started offering live demos and is actively looking for partners to onboard. The team is currently prioritizing programs focused on housing, utilities, and food, though they remain open to onboarding other types of services. Ongoing updates will continue to be shared on the dedicated SRN webpage: unitedwaytc.org/srn.
Kings County SRN Update Maria Valdez, SRN Coordinator for Kings United Way, celebrated a major milestone: the SRN program has officially launched in Kings County. The network is already seeing strong engagement, with 12 active agencies currently utilizing the platform and 38 active users responding to referrals to get community members the help they need.
Spotlight on Community Partners
Kaweah Health Savanna Vigil and Janelle Brodbeck showcased Kaweah Health’s Enhanced Care Management (ECM) and Community Supports programs under CalAIM. Their holistic, patient-specific approach addresses social determinants of health for vulnerable populations, including those experiencing homelessness, dealing with complex medical diagnoses, or managing serious mental health disorders. A major highlight was the success of their housing transition team, which has successfully housed over 114 patients, families, and their pets since the start of 2024.
Zocalo Health Lupe Delgado introduced Zocalo Health, a virtual clinic providing culturally aligned primary care, telehealth behavioral services, and ECM support tailored for the Latino community. By utilizing promotores (community health workers), Zocalo Health removes traditional barriers to care and meets patients where they are most comfortable. Lupe also highlighted their innovative "Cafecito" events—community gatherings featuring music, food, and health education designed to build grassroots trust and foster organic referrals.
Feature Highlight: Cross-County Referrals Fedieson provided a demonstration of the SRN’s new cross-county referral feature, a major step forward for regional service delivery. The platform now allows users to easily search for and seamlessly refer clients to organizations that serve both Kings and Tulare County residents, breaking down geographical barriers and expanding resource opportunities for families across the Central Valley.
Looking Ahead The meeting concluded with a reminder that agencies serving the region can be added to the 211 directory and SRN, even if they are physically headquartered outside of Kings or Tulare counties.
The Kings/Tulare SRN Alliance continues to demonstrate the power of regional collaboration. With each meeting, partners strengthen their networks, share resources, and move closer to a fully integrated referral system.
