Kings-Tulare Smart Referral Network Alliance Meeting | August 20, 2026
On August 20, 2026, community partners, healthcare providers, and local advocates gathered virtually for the monthly Kings/Tulare Smart Referral Network (SRN) Alliance Meeting. Facilitated by Maria Valdez and Fedieson Landicho, the session focused on expanding regional access to healthcare, whole-person care coordination through CalAIM, and strengthening closed-loop community referral pathways across Kings and Tulare counties.
Agency Spotlight: Omni Family Health
Pedro from Omni Family Health opened the meeting by highlighting the organization’s 45-year history of providing comprehensive, accessible care to agricultural workers and rural communities across Kern, Kings, Tulare, and Fresno counties.
Key Services & Enrollment Support
Comprehensive Care: In addition to primary and preventive care, Omni provides dental, pharmacy, behavioral health, women’s health/OB-GYN, optometry, chiropractic, and substance use services.
Bilingual Certified Enrollment Counselors: A dedicated team of 13 bilingual navigators assists community members Monday through Friday with Covered California, Medi-Cal applications, and renewal packets.
Sliding-Fee Scale Program: For individuals who do not qualify for Medi-Cal or Covered California due to immigration status or other barriers, visits are available for as low as $45 with no immigration status requirements.
Mobile Health Unit: Omni’s mobile health bus collaborates with school districts, non-profits, and health fairs to deliver general physicals, sports exams, and immunizations directly into underserved communities.
Agency Spotlight: Exceptional Parents Unlimited (EPU)
Cynthia Ruiz and Daisy Perez Rios, Lead Care Managers at Exceptional Parents Unlimited (EPU), presented on their expanded CalAIM Enhanced Care Management (ECM) and Community Supports programs in Kings County.
Whole-Person Care & CalAIM Integration
50 Years of Family Advocacy: EPU supports children, youth, and families navigating complex developmental, behavioral, medical, and social needs across five Central Valley counties.
Community Health Worker (CHW) Workflow: CHWs establish trust through in-person engagement, complete Social Determinants of Health (SDOH) screenings within 48 hours of referral, and connect families to lead care managers.
Enhanced Care Management (ECM) & Housing Support: EPU provides intensive care coordination, multidisciplinary team case conferencing, and housing navigation for families with children (ages 0–17) facing housing instability or eviction risk.
SRN Alignment: EPU leadership is actively reviewing integration with the Smart Referral Network to facilitate seamless, bi-directional referrals with regional partners.

Alliance partners shared several upcoming community events and collaboration opportunities across the South Valley:
KCAO Grizzlies Night & Monte Carlo Fundraiser: Kings Community Action Organization announced their Fresno Grizzlies partnership game on September 4th and their 2nd Annual Monte Carlo Masquerade Night on November 6th.
Family HealthCare Network Health Fairs: FHCN is hosting two upcoming community health fairs—September 24th at Zumwalt Park in Tulare and October 18th in downtown Hanford.
United Way of Tulare County Annual Breakfast: UWTC’s Annual Recognition Breakfast takes place on September 26th to celebrate community contributors and program impact.
UCP Trunk or Treat: United Cerebral Palsy will host its annual community Trunk or Treat on October 10th.
Looking Ahead
The next Kings/Tulare Smart Referral Network Alliance Meeting is scheduled for Thursday, September 17, 2026. The September meeting will feature an interactive SRN Partner Panel, offering service providers an inside look at closed-loop referral workflows and best practices in action.
